A referral conversation should help clarify the person's current circumstances, assessed needs, risks, desired outcomes, and the most appropriate pathway.
Name, age, current living arrangement, local authority area, tenancy or accommodation position, and who should be involved.
Learning disability or autism diagnosis, personal care needs, medication, safeguarding, behaviours that challenge, health vulnerabilities, and crisis risks.
What the person wants life to look like, what independence skills they want to build, and what support would help them feel safe and settled.
From the first form to a decision on next steps, the process is designed to be honest, quick, and free of unnecessary back-and-forth.
